Drug confirmation screens are required for all non-negative instant drug tests.

Please select the relevant pathology referral form for your Clinic from the below list. Please note that each form is unique, so it is important that you download the correct form.

The closest collection centres to your clinic have been listed in the referral form. As not all laboratories perform confirmation drug testing, it is important that the candidate attends the correct pathology collection centre listed in the referral form. 

Please fill in the candidates’ details, including name, date of birth and address.

If you have any queries please contact your Clinic Relationship Manager.

Please advise candidate:

  • They MUST present for a confirmatory drug test at our pathology collection centre immediately after the non-negative instant drug test. Failure to do so may result in their assessment being incomplete, which may impact their application. If they are unable to attend immediately, they must contact Bodycare to advise.
  • Bodycare may contact them within 3 hours of their appointment time to check if they have attended.
  • If they attend a different collection centre to that listed on the referral form, they may be invoiced for the service, rather than the account being sent to Bodycare.
